Do I need a referral from my doctor?PPO/Private Plans: No - many insurance plans allow for an initial evaluation by a physical therapist without a doctor's referral. Medicare: No - however, federal regulations require your physician's signature on the plan of care following your first visit. This will be faxed for them to sign after your initial evaluation. HMO Plans: It depends - some plans require prior authorization before getting started. Please call with your insurance details to verify your benefits. How much will treatment cost?You may be fully covered, you may owe a copay/coinsurance, or may have to pay toward your deductible. That being said, because everyone's insurance benefits vary, Movement Restoration Physical Therapy will provide a FREE insurance verification check. If you wish to a elect direct or cash pay option, initial evaluations are $180 and subsequent visits are $140/session.
